Please Note:
- For health card registration, the wife of a citizen must be sponsored by her husband and registered in his Family Book.
- In the event of the husband’s demise or divorce, a health card cannot be issued even if the woman is sponsored for residence by her sons, who are residents.
WHAT ARE THE DOCUMENTS REQUIRED FOR THE HEALTH CARD?
These are the documents needed to get a health card in Sharjah:
- A recent passport-sized photograph with a white background.
- A copy of your Emirates ID (both sides) or a copy of the ID application form.
- Proof of residence (lease contract, property ownership contract or electricity bill).
- Birth certificate (for kids).
- Marriage certificate (for sponsored spouses).
Other than the above-mentioned documents, some specific documents are required from certain groups of applicants.

HOW TO APPLY FOR THE HEALTH CARD IN SHARJAH?

Online Process
- Visit the EHS official website and register yourself for the UAE PASS
- Login to the e-services portal
- Select your form type
- Enter your specifics in the required fields
- Pay the Sharjah health card fees (details below)
- Your application will be reviewed at one of the EHS service centres in Sharjah
- Once approved, you will be notified about it via an SMS, and you can enjoy the health card’s benefits in Sharjah.

- Sharjah no longer issues an actual physical health card. Rather, it is an electronic card and is linked to your Emirates ID.

- Only one card is issued per person.
